Yes these side effects are real. Dutasteride is a two-edged sword. On one hand it has shown to slow the progression of prostate cancer...but on the other it can raise your risk of getting high-grade prostrate cancer. Probably from masking the biological markers of it in lab tests until it's too late and well-established and harder to deal with. A topical made from these drugs might be a safer option if they can absorb well.

Via the pathways it would seem that suppressing DHT increases cancer risk or possibly by altering the steroid pathway for progesterone.
